
Home and Community-Based Services
The Home and Community-Based Services include residential services, adaptive aids, day habilitation (help with the development of basic life skills), minor home modifications, nursing, respite, professional therapies, supported employment, and other services.
HCS also offers people the choice to self-direct some services. This means anybody enrolled in the program can have control over how some of their services are delivered and managed.
Home and Community-Based Services (HCS) provide opportunities for individuals with intellectual or developmental disabilities, physical disabilities, and/or mental illnesses to receive services.
Texas Home Living (TxHmL)
At Trinity Care would provide our participant with Texas Home living service to those who qualify. The Texas Home Living (TxHmL) is a Medicaid waiver program that provides essential services and supports to individuals with an intellectual disability (ID) or a related condition so that they can continue to live in the community. This service is available to any individual resident living in his or her own home or family’s home and have.
- Has an approved documentation of Intellectual disability or related condition
- Has mild to severe deficits in adaptive behavior.
- Is eligible for Medicaid benefit
- Is not enrolled in any other Medicaid waiver program.
